It is likely Equinor' s fault which supply a erroneous fatigue analysis program and the hull had passed various Equinors internal inspections previously. But Equinor is the customer which Sembmarine is trying to win the Witsing FPSO deal in the coming months based on the proprietary cylindrical hull. Therefore, Sembmarine largely keeping silent and helping Equinor to deflect some heat from the Norwegian regulatory authority. That' s the back story if you put 2 and 2 together.

These include the fact that the contractor (SCM) failed to provide sufficient competent welders and welding inspectors for execution of the scope and did not sufficiently comply with processes and work routines for execution and follow up of welding and inspection activities. Furthermore, Equinor has not sufficiently complied with its own work processes and procedures for risk management, construction management, and follow-up at the construction site."
